I need help coming up with a Nursing POC using the NANDA.

Risk for infection R/T (pathophysiology) secondary to (medical diagnosis)

I need help coming up with a Nursing POC using the NANDA. The pretend patient I am making it for doesn’t have very much information listed. They have osteoporosis and a right hip fracture that was corrected via hip replacement surgery. They have slow wound healing and are having dressing changes BID and were said to have surgical wound complications. So I have been assigned to write a plan for the nursing diagnoses of “risk for infection”. I am having trouble coming up with the secondary to (medical diagnosis) for this. Can a medical diagnosis be “right hip replacement surgery”? Would this be correct treminology? I thought of putting osteoporosis, but I don’t think that would increase the risk for infection. Or would “surgical wound complications” be a medical diagnosis? HELP please
