Title Page: should include title of project, name, course, date
Proposal: should include following subtitles; page counts are approximate but should be adhered to as much as possible. Final proposal should be 10 – 12 pages, including references.
Introduction/Background: This should be a summary of the who, what, why and how of your proposal – just the highlights. (1 page)
Statement of need: Why is this project needed? This section should include specific data from reliable sources. Make sure and cite. You can include tables or graphs. (1 page)
Goals and Objectives: The results that your project seeks to achieve. Should be written using SMART format. No more than 1 goal and 2-3 objectives (1/2 – 1 page).
Implementation Plan: The activities you will carry out to achieve your goals and how you will carry them out. (2 – 3 pages)
Evaluation Plan: The methods you will use to measure your progress toward your outcomes. If you use instruments created by someone else, cite appropriately. (1/2-1 page)
Timeline: A chronological description of key project activities. Use table or graph such as grant chart. Grant is 12 months. (1 page)
Budget: A table or excel spreadsheet of your project budget (up to $100,000). (1/2 – 1page)
Budget Narrative: Each line item should be explained in a narrative. (1 page)
Logic Model: Use template provided. (1 page)
Resources: List of citations (1/2 – 1 page)
