Define the concepts of health disparities and social determinants of health.

Define the concepts of health disparities and social determinants of health.

Healthy People 2020 (2014) defines Social Determinants of Health as, “conditions in the environments in which people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks” (U.S. Department of Health and Human Services, 2014). “Many racial and ethnic minority patients experience lower quality health care compared to white patients” (University of Chicago & RWJF, 2014). Disparities in health outcomes exist not only across racial and ethnic groups, but can be related to sexual identity, age, disability, socioeconomic status, and geography as well (USDHHS, 2014).

This week’s learning materials included an online course (IHI Open School: Improving Health Equity), and videos on innovative ideas to address and close the gap in health disparities by addressing the social determinants of health (beyond health care). Additional resources in this week’s module highlight private industry and public (government) efforts to shine a light on disparities when they exist, and provide examples of programs and services to reduce or eliminate disparities–examples at individual, organization and population levels.

As you gather information on your Community Health Assessment assignment, take note of any disparities noted in your data (Review the Economic Indicator, Education Indicator and Health Equity reports in Community Commons). Consider the following questions in developing your initial post to this discussion board:

Define the concepts of health disparities and social determinants of health.
Introduce your county (for Community Health Assessment paper).
Describe disparities that you found to be present for at least two health conditions for your county.
Identify and describe at least one innovative program to address disparities (from the course materials/videos/readings). What are the desired results of that program? What are the factors which will impact it’s success?
Propose at least one strategy to address the identified health disparities within your community. Who would need to be involved in order for this strategy to be successful?

Use and cite resources and research articles; be creative and share your thoughts and ideas! Respond to at least one of your peer’s postings.

Identify the independent and dependent variables in the study.

Identify the independent and dependent variables in the study.

This week builds on that foundational awareness with a focus on the application of evidence-based practice models as a strategy to improve patient safety and other quality dimensions. In this Discussion, consider how these strategies can sustain practice changes.
To prepare:
Read: Newhouse, R.P. (2007) Diffusing confusion among evidence-based practice, quality improvement and research. JONA 37,432-535 (see attached pdf)
Read: Mazurek Melynk B., Gallagher-Ford, L., English Long, L., & Fineout-Overholt, E. (2014) The establishment of Evidence-Based practice competencies for practicing registered nurses and advanced practice nurses in real world clinical settings: proficiencies to improve healthcare quality, reliability, patient outcomes, and costs. Worldviews on Evidence-Based Nursing 11(1),5-15. (see attached pdf)
We all agree that research is the highest form of reliable evidence, when implemented in an organization that provides the basis for an evidence based practice. FIND a research study published in a peer reviewed journal related to a clinical practice problem that is of interest to you, and which would ultimately contribute to an evidence based practice.
The discussion assignment:
ANSWER the following discussion questions. Include the research study in your post (attach it to the post and post it in doc sharing). See page 11 #15 in Mazurek Melynk, Gallagher Ford, English Long and Fineout-Overholt. In this assignment, you are being asked to critically appraise a single research study for its relevance to a QI practice problem.
Discussion questions:
1) What was the purpose of the research?
2) Identify the independent and dependent variables in the study.
3) Briefly describe the research design, data collection method(s), and instruments used to measure the variables under study.
4) Briefly summarize study findings, conclusions and recommendations. Do you agree with these?
5) As a DNP prepared nurse, would you recommend a change in nursing practice based on the study? Defend and/or justify your decision based on research evaluation principles. In other words, does the evidence generated by this research article signify a need to change nursing practice? If not, why not; if so, why?

Describe the disorder you selected from the Women’s Health Initiative study.

Describe the disorder you selected from the Women’s Health Initiative study.

For over 20 years, the Women’s Health Initiative (WHI) study has conducted research trials to examine factors that impact risks and development of conditions that impact women such as heart disease, breast and colorectal cancer, and osteoporotic fractures in postmenopausal women. Health care providers use results from these trials to develop guidelines for delivering care to patients. In your role in clinical settings, you must be familiar with these guidelines, and you must be aware that through clinical practice, guidelines frequently evolve and change. Often, what is considered a best practice today might not be a best practice in the future. For this Assignment, you compare guidelines outlined in the WHI study to current best practices for assessing and managing conditions.
To prepare:
-Review the Women’s Health Initiative article in this week’s Learning Resources. Select one disorder presented in this study
Department of Health and Human Services, National Institutes of Health, & National Heart, Lung, and Blood Institute. (n.d.). Women’s Health Initiative: WHI background and overview. Retrieved March 20, 2013, from https://www.nhlbi.nih.gov/whi/background.htm
The disorder I choose I choose is: Breast Cancer Risk (April 2006)
-Locate and select a research article that addresses current best practices for assessing and managing the disorder you selected. Be sure that the research article you select is from a reputable source.
Consider the similarities and differences between the best practices presented in the WHI study and the article you selected ( I will attach the article). Think about the potential impact of differences in best practices on women’s health.
Consider whether the best practices in the article you selected should be used in clinical practice.
To complete:
Write a 2 -page paper that addresses the following:
Describe the disorder you selected from the Women’s Health Initiative study.
Explain the current best practices for assessing and managing this disorder as suggested in the research article you selected.
Compare the best practices presented in the WHI study to the current best practices presented in the article. Explain how the differences in best practices might impact women’s health.
Explain whether you think the current best practices in the article you selected should be used in clinical practice. Support your position with evidence-based research.

Assignments must be submitted as a Worddocument. If I cannot readily read your format, then it will be returned with a note and a score of zero. PDFs and “screen shots” are not acceptable since they do not allow ready application of comments on your submission. The submission must be double-spacedwith 11 or 12 inch Times Roman font. You must use APA Stylefor parenthetical citations and full references for the parenthetical citations.

You acknowledge that the work that you submit is your own. Plagiarism earns an F on the assignment. A second offense earns an F for the course. This will put your continuation in the program in jeopardy. Public health clients trust us. I trust you. If you violate that trust, how can I trust you to place people into your care? Trust is hard won. Once it is lost, it is extremely difficult to restore. Your word is your bond. Trust is the “coin of the realm” in academia, professional, and community life. I strongly urge you to make use of the Writing Center if there is any doubt about plagiarism. You have resources online at Purdue OWL for plagiarism and APA style.

Your weekly submission must demonstrate that you have read the material. One sure way to do this is by citing pages from the readings where appropriate to support the claims that you make in your submission. I would expect several different citations from the readings. Plus, I would expect at 2 citationsfrom the peer-reviewed research literaturefor each questionin the assignment. If there are two questions, then a minimum of 4 peer-reviewed research citations is required. You’re in graduate school so the expectation is that you read and become familiar with the relevant literature to inform your understanding. It’s a professional expectation. Get help from the Writing Center or Research Librarian if you don’t know how to review the literature and incorporate your findings into your submission.

What is the difference between a quality improvement system and a tool? Provide examples of each.

Assignment 2: Health Policy Presentation

The purpose of this assignment is for you to identify an issue of concern for your role as an advanced practice nurse and to formulate a potential policy change to address that issue. There are many potential issues which can influence your practice setting or other issue which may negatively affect the patients with whom you work. All of the course reading will help you to identify a topic for this assignment. You can think about the issue as related to your health promotion project. The policy you consider may be in reaction to the health promotion issue or something larger that is still related to that issue. There are hundreds of possible issues, but here is a list of a few to consider:
•Child and elder care
•Civil rights
•Domestic violence
•Drug abuse/addiction
•HIV/AIDS
•Homelessness
•Native American and migrant workers’ health
•Long-term care
