Please use current and evidence-based scholarly (peer reviewed) references to support the discussion, writing in academic (third person) language, (except when integrating personal reflections, as outlined below, where first person is appropriate). A minimum of 10 scholarly references must be used.You will need to:? Outline your case study’s scenario and circumstance (this should be person-centred and short – only approximately 250 words – remember to ensure confidentiality of the practice setting and use a pseudonym for your client).? Discuss assessments performed, identify problems and construct an appropriate plan of nursing care that is person-centred, age-appropriate and culturally sensitive.? Highlight any risks identified during the assessments performed (if assessed and no risks identified, discuss this).
